1Do you take health insurance?

I am not currently paneled with any insurance companies, however I can provide you with a superbill that includes appropriate ICD (medical diagnostic codes) and CPT (procedure codes) to submit to your insurance for partial reimbursement if you have out of network benefits. On the first of the month, a superbill statement will be generated which can be downloaded from your secure portal account.

Prior to scheduling your first appointment, please know what your out of network mental health benefits are. If you are not sure about your benefits, please contact the member services number on the back of your insurance card. It should only take a few minutes. You will want to verify the following information:

1) What are my out of network mental health benefits?

2) What is my reimbursement rate?

3) What is my session allowance?

4) Do I have a deductible? Note: a deductible is a specified amount of money that the insured must pay before an insurance company will pay a claim.

-Do I have to meet the deductible before insurance will cover the therapy session?

-What is the deductible amount?

5) What administrative steps do I have to take to get reimbursed?

2How much do your services cost?

For fee schedule, please email me. https://www.drhannahdacruz.com/contact/

3What payment do you take?

I only accept cash, credit or debit cards as payment at time of service. Health Savings Account (HSA) and Flex-Spending Account (FSA) credit or debit cards may also be used as payment.

4How often does therapy occur?

While the length and duration of sessions depends upon presenting concerns and needs of the individual or family, sessions typically occur weekly for 50 minutes.

5Can we talk before meeting in person?

Absolutely. Prior to meeting in person, it can be very helpful to briefly speak over the phone (approx. 15 minutes) to discuss your reasons for seeking services, address any questions you have for me and mutually decide whether to proceed with scheduling an in-person appointment.

6How can I make an appointment?

If you would like to talk about working together, or learn more about what to expect, contact me by completing the form provided here https://www.drhannahdacruz.com/contact/ or email me directly at drhannahdacruz@gmail.com.

7Do you provide telehealth or remote sessions?

Yes. I provide video therapy, coaching and groups for individuals residing anywhere in California. I use a secure platform online for video sessions.
